About the role

  • As an NDIS Support Coordinator, you will help participants navigate the NDIS system, ensuring they access the right services to achieve their goals. Responsibilities include coordinating supports, liaising with providers, assisting with plan implementation, and NDIS reporting. You will provide guidance, advocacy, and educate participants on their rights. The ideal candidate will have strong communication and budgeting skills, a passion for helping others, and a solid understanding of the NDIS framework.

Your primary responsibilities will include

  • Manage a portfolio of participants & coordinating supports to assist individuals with disabilities
  • Budgeting, track, and adjust participants’ fund usage to ensure effective management
  • Support participants in making choices and accessing a range of funded and community-based services, ensuring options are consistently available
  • Collaborate with key stakeholders to align on coordinating service delivery and building effective connections to support the participant
  • Submit NDIA mandatory reporting at specified due dates
  • Organize & attend meetings with clients, their families, and providers, ensuring prompt follow-up on any pending actions
  • Understand the participant’s environment and address any crises that affect the participant’s daily routine
  • Work within the varying funding and service levels of Support Coordination to meet the specific needs of each participant
  • Document daily case notes to track progress and ensure accurate, timely billing
  • Knowledge of online systems and software (Microsoft Office, Oracle, Outlook) to efficiently record and monitor participant activity
  • Prioritize tasks, manage your time, multi-tasking and organize work to ensure high-quality outcomes
  • Promptly inform management of any issues, incidents and ensure proper reporting procedures are followed
  • Assist less experienced team members, and provide leave coverage as needed.
  • Engage in monthly & annual performance reviews, and required internal and external training

What We're Looking For

  • Excellent communication skills, both written and verbal – Ability to clearly communicate with participants, families, providers, and other stakeholders
  • Knowledge of NDIS Framework – Understanding NDIS processes, policies, and available support to assist participants
  • Problem Solving Ability – The capability to address challenges and find practical solutions for participants
  • Attention to Detail – Ensuring all participant information, plans, and documentation are accurately recorded and managed
  • Organizational Skills – Efficiently managing multiple tasks and appointments, prioritizing duties as needed
  • Time Management – Ability to meet deadlines and manage a caseload effectively
  • Collaboration and Teamwork – Working effectively with other professionals and stakeholders to achieve participant goals

  • 100 Points of Identification
  • A valid Driver’s License and comprehensively insured motor vehicle
  • Full Australian working rights
  • An Australian Business Number (ABN)
  • A valid NDIS Worker Screening Check
  • Your State’s Working with Children Check
  • Current national police check
  • Copies of relevant course certification
  • First Aid / CPR certificate
